1. Please satisfy all safety training requirement through UCLA EH&S, specifically the Chemical Hygiene Plan, Laboratory Safety Fundamentals, and Fire Extinguisher Training.
2. Users in the department of Chemistry and Biochemistry: Please obtain your chemistry department recharge account from your research group.
3. Users from all other departments: Please request a chemistry department recharge account (3 letters followed by 3 numbers) by completing and emailing this form to Sawako Leslie.
4. Sign up for training with the relevant MIC instrumentation facility.
